Any protected health information (PHI) that is created, stored, transmitted, or received in any electronic format or media is considered to be ePHI. Common examples of ePHI are names or addresses of patients. Examples of less common but equally sensitive ePHI could be a patient’s medical record number or health plan beneficiary number.

2: Protect ePHI systems and data against reasonably anticipated threats.

Businesses that are obligated to be HIPAA compliant include health care providers (covered entities) and health care vendors or IT providers (business associates). These businesses must implement an effective HIPAA compliance program that addresses the HIPAA security requirements listed above.
